Chief Executive Officer vs. Chief Operating Officer
What's the difference?  Generally speaking, CEO's are charged with figuring out where the company is going - essentially finding a destination that fulfills a particular vision.  The typical CEO has great creative abilities that are supported by sound strategic reasoning.  To compliment this creativity, there is the COO.  Their job is largely based on figuring out how a company get's to that chosen destination.  The COO's toolbelt is usually filed with methodical planning, tactical insight and assertive diligence to will the company forward.  If it seems like these are two different skill sets - you're right.  

Larger companies can usually afford the benefit of both positions.  Small businesses, particularly those in the early phases of existence, simply can't.  Consequently, all of the responsibility falls on the owner's shoulders.  More often than not, the owner is either a visionary or a subject matter expert that is focused on execution.  Rarely, does a small business enjoy the luxury of an founder who is skilled at both.  As a result, the business is either great at strategy but lacking in execution or great in execution but lacking strategy and vision.  Either way, it is out of balance and will need to adjust to survive.
